• Online Address Changes – Licensees can make changes to their personal address through the Online Services section of the Board website and have it update LLR’s database. Changes in employment address must be submitted on a change of employment form available for download on the Board of Pharmacy website. Facilities must submit either a relocation application or a new permit application for a change of address.

Attention LLR Customers

LLR encourages the use of its online licensing services to issue licenses as quickly as possible to our clients. Starting May 15, 2011, a $25 service fee will be instituted for licensing services done in person rather than online.
