South Carolina Board of Physical Therapy

License Renewal Requirements/Reinstatement

All physical therapists and physical therapist assistants must renew their licenses biennially on or before December 31 of every other year. Renewal application forms are mailed to all current licensees in early November. The next renewal period will be December 31, 2008. At that time, licensees will be required to report 3.0 CEUs (30 hours of continuing education) which will have been earned between January 1, 2006 and December 31, 2008. See Continuing Education.

If there is an address or name change at any time during the two-year period, notification is to be sent to the board office within 30 days of such change. A copy of the legal document effecting any name change is required as proof before the Board can change its records.

I. License Renewal Requirements

Complete all questions on the application, sign and return with any accompanying explanations and fee. See Fee Schedule. Incomplete applications will be returned for corrections.

A person who fails to renew a license and who practices as a physical therapist or a physical therapist assistant after December 31 is practicing without a license and is subject to penalties. If a licensee fails to renew by January 31 the license will automatically lapse.

A licensee who does not wish to renew an active license may select inactive status on the renewal form and return the form to the Board office no later than January 31.

II. License Reinstatement Procedure

By law, no person shall practice as a physical therapist or physical therapist assistant without holding an active license in this state. Licenses not renewed or placed on inactive status as of January 31, 2009, are considered lapsed. The reinstatement procedure is as follows:

A PT/PTA who is not currently licensed in another state and whose S.C. license has lapsed or been inactive for:

(1) Fewer than three years may reinstate the license by making application to the Board, paying the reinstatement fee, and submitting proof of 1.5 CEUs for each full year that the license has been lapsed or inactive;

(2) At least three years, but less than five years, may reinstate the license by making application to the Board, paying the reinstatement fee, and satisfactorily completing 1,000 clinical practice hours under the on-site supervision of a S.C. licensed physical therapist;

(3) At least five years, but less than ten years, may reinstate the license by making application to the Board, paying the reinstatement fee, satisfactorily completing 1,000 clinical practice hours under the on-site supervision of a S.C. licensed physical therapist, and successfully completing a course(s) approved by the Board;

(4) at least ten years or more may reinstate the license by making application to the Board, paying the reinstatement fee, successfully passing an examination approved by the Board, and satisfactorily completing 1,000 clinical practice hours under the on-site supervision of a S.C. licensed physical therapist.

A PT/PTA who is currently licensed in another state and whose S.C. license has lapsed or been inactive for:

(1) Fewer than three years may reinstate the license by making application to the Board, paying the reinstatement fee, submitting proof of completing 1.5 CEUs for each full year that the license has been lapsed or inactive, and submitting official verification of a current license;

(2) Three or more years may reinstate the license by making application to the Board, paying the reinstatement fee, submitting evidence satisfactory to the Board of not less than 1,000 hours of out-of-state employment within the last year and official verification of a current license. No application is complete without the required documentation and non-refundable reinstatement fee payable to SCBPTE. See Fee Schedule. Personal checks are acceptable. A $20 charge will be imposed for checks returned for insufficient funds. If there has been a change in name, a copy of the legal document (marriage license, divorce decree, etc.) is required. The document does not have to be certified.

Provisional Licensure

A provisional license may be issued to a licensee whose physical therapist or physical therapist assistant license has been inactive or lapsed for three years or more and who has applied to reinstate the license. The provisional license is valid for one year from the date of issue, may not be renewed, and authorizes the individual to work in order to obtain the 1,000 clinical practice hours to reinstate the license. The physical therapist provisional licensee must work under the on-site supervision of a South Carolina licensed physical therapist. A physical therapist assistant provisional licensee must work under the on-site supervision of a South Carolina licensed physical therapist or physical therapist assistant. A provisional licensee shall submit to the Board a completed supervisory agreement form for each place of employment
