Attention LLR Customers

LLR encourages the use of its online licensing services to issue licenses as quickly as possible to our clients. Starting May 15, 2011, a $25 service fee will be instituted for licensing services done in person rather than online.

Certification Fee - $25

Biennial Renewal Fee - In State Optometrist - $230

Biennial Renewal Fee - Branch Office - $230

Biennial Renewal Fee - Out of State - $230

Late Fee - $50

Reinstatement Fee - $100

License Verification -$5

Duplicate Pocket Card - $10

Name Change and New Pocket Card - $10

List of Licensed Optometrists - $10 Licensee List Request
