Complete the Licensure List Request Form and return to the Board with appropriate fee.

What information is include in the Licensee List Request?

Last name, first, middle, profession, license number, business address, business telephone number, license issue date, license expiration date and Specialty.

What format does the licensee information come in?

Microsoft Excel.

What is the turnaround time after I mail in my request?

Two weeks.

Can you email me the list?

Yes, provide your e-mail address on the Licensure List Request Form.

Does the listing come on labels?

No. However, you can print the listing onto labels.

Can I get listing for certain counties?

Yes.

If I bring the request form and fee to your office, can I get the listing the same day?

Yes, contact the Board office in advance at (803) 896-4500 with the type of listing and we will have it waiting at the receptionist’s desk.
