Time to Update Your Contact Information with the Board
With the 2015 renewal season approaching, now is a good time to make sure all your contact information with the board is up to date so we can keep you informed.
S.C. Code 40-47-41 (C) of the medical practice act requires that licensees shall notify the board in writing within fifteen business days of any change of residential address, office address, or office telephone number.
In addition to your residential address, office address and office telephone number, it is also very important that you let us know if your name has changed, or if you have a new e-mail address, home address or contact phone number.
For your convenience, it is not necessary to call us to update your information.
You may update your address at the board’s website http://www.llr.state.sc.us/POL/Medical/ under ‘Online Services’.
You may e-mail other changes to firstname.lastname@example.org
Please note: for a name change, a copy of the legal document showing the name change must be provided.
