License Instructions
The following requirements are for applicants who passed the National Massage/Bodywork Therapist Examination or the Federation of States Examination
A. Applicant must be 18 years of age or older.
B. Have a High School diploma or Graduate Equivalency Diploma (GED)
C. Completed a 500-hour course at an approved massage/bodywork school.
D. Passed the National Certification Examination or Federation of States Examination for Massage/Bodyworks. (Any examination other than the NCE or the Federation of States Examination must have prior Board approval.)
The following forms need to be completed and sent back to this office:
1. Application for license for massage/bodywork therapist.
a. Complete items 1 through 19. Sign and date the application.
b. Have the application notarized and mailed back to this office.
2. Certified true copy of your massage/bodywork therapist school certificate or transcript.
3. Certified true copy of your National Examination Certificate or Federation of States Examination.
4. Check or money order in the amount of $150 to:

All applications will be returned if not properly completed or fees not enclosed.
