Fees

LLR encourages the use of its online licensing services to issue licenses as quickly as possible to our clients. Starting May 15, 2011, a $25 service fee will be instituted for licensing services done in person rather than online.

The following non-refundable fees have been established by the Panel for Dietetics:

A. Application: $175

B. Biennial Renewal of License: $100  Late fee: $50

C.  Reinstatement Fee + Late Fee: $100

D. Miscellaneous administrative charges:

Replacement of wall certificate: $15

Duplicated pocket card: $10

Name Change and New Pocket Card: $10

Fee for returned checks: $30

Licensure Verification: $5
