How to Apply for Licensure

Applicants for licensure, registration or certification must complete an application and submit it along with the applicable fee to the Board office. Application fees are payable by a cashier’s check or money order made payable to: LLR - Board of Dentistry. No refunds are made after an application and fee have been filed with the Board. Questions concerning licensure / registration / certification requirements in South Carolina should be directed to the Board office at (803) 896-4501.

Application are valid for one (1) year from the date the application is received in the Board office. Results of the SC Dental Practice Act (DPA) Examination are considered part of the application and are, therefore, valid for one (1) year from the date the application is received in the Board office. After one (1) year, an application, including DPA exam results, an application, including DPA exam results, is considered null and void. Results of dental and dental hygiene clinical licensure examinations (*SRTA/ADLEX?ADHLEX) are valid for five (5) years.

  • SRTA Dental Examination 1/1/96
  • SRTA Dental Hygiene Examination 1/1/97
  • ADLEX (CRDTS / NERB) Dental 1/1/06
  • ADHLEX (CRDTS / NERB) Dental Hygiene 2/24/06
