Continuing Education Sponsor - Permanent

The application form must be completed and returned to the Board office with a nonrefundable application fee of $150. You can apply for permanent sponsorship after you have previously completed three one-time Board-approved CE offerings. The Board will review your application and you will receive the results shortly thereafter.

Permanent sponsorships are renewed biennially on November 1st. Renewals are contingent upon Board review of:

• Biennial update form
• Payment of the renewal fees
• Evaluation form and summary of the evaluations for one of your activities
