How to Apply for a License
Application Forms:
You may download documents directly from the internet or contact the Board office for an application packet.
Give your complete postal mailing address, type of application requested. Requests for applications should be directed to:

Procedure for Applying for Licensure:
General Information
No application is complete without all of the required documents and fees. The application is not acceptable if any portion has been executed and dated prior to one year before filing with the Commission. All documents will be retained as a permanent part of the applicant's file.
Applicants submit the following:
All Documents Must Have Original Signatures. Faxes Are Not Acceptable.
1) Original application with all sections completed by applicant and Doctor.
2) Original Event Permit with all section completed by promoter and notarized.
3) Check or money order, payable to S.C. State Athletic Commission. A $30 fee will be charged for checks returned for insufficient funds.
